Checklist of Items Required

1. A Clinical Affiliation Agreement (If not already established)
2. Supervising Licentiate Attestation
3. Clinical Preceptor Information
4. Facility Information
5. Facility Radiation Protection Program Information
6. Fluoroscopy Information (if applicable)
7. Photos of “Caution X-ray” signs are posted on all public entryways to all X-ray rooms.
8. Provide documentation of staff access to the current printed or electronic copy of Title 17.  Submit a picture or scan of the pages with section 30417.
9. Provide a current “Notice to Employees” and submit a picture of the actual posted notice.
